Tsiupryk N. Effectiveness of endovascular and hybrid revascularization of stenotic-occlusive lesions of the infrainguinal arterial bed in conditions of concomitant type 2 diabetes mellitus

The studies were focused on studying the influence of pathophysiological disorders in stenotic-occlusive atherosclerotic lesions of the arteries of the lower extremities in type 2 diabetes mellitus, which is reflected in changes in the indicators of the blood coagulation system, acid-base status, degree of endothelial dysfunction, changes in the level of arterial blood flow in the distal arterial bed, both at the preoperative stage and after surgical restoration of blood flow in the infrainguinal arterial bed. The obtained results of the scientific search allowed to supplement the knowledge on the issues of chronic arterial ischemia of the lower extremities in patients with atherosclerotic stenotic-occlusive lesions of the arteries of the infrainguinal segment of the lower extremities in conditions of concomitant type 2 diabetes mellitus and to propose optimal tactics in choosing revascularization of the infrainguinal segment and treatment of postoperative thrombotic complications. The risk level of postoperative complications was determined based on multiparametric neural network clustering in atherosclerotic stenotic-occlusive lesions of the arteries of the infrainguinal segment of the lower extremities in conditions of concomitant type 2 diabetes mellitus when performing endovascular and hybrid reconstructive interventions. The impact of metabolic changes was assessed, the state of endothelial function and the level of systemic inflammatory response were investigated in patients with atherosclerotic stenotic-occlusive lesions of the arteries of the lower extremities in conditions of concomitant type 2 diabetes mellitus when performing endovascular and hybrid reconstructive interventions. The degree of arterial blood flow disturbance in the distal sections of the lower extremities and the degree of acid-base metabolism disturbance in atherosclerotic stenotic-occlusive lesions of the arteries of the lower extremities in conditions of concomitant type 2 diabetes mellitus were investigated when performing endovascular and hybrid reconstructive interventions. The effectiveness of endovascular and hybrid revascularization methods was assessed in patients with atherosclerotic stenotic-occlusive lesions of the arteries of the lower extremities in conditions of concomitant type 2 diabetes mellitus.
